In this request for an application, you will need to provide your name and email address, faculty advisor name and email address, and department chairperson name and email address. The faculty advisor is the professor who you are working with to complete the Honors Project--they are the instructor teaching the class in which you are completing the project. Please be sure there are no typos and you have looked up the correct department chairperson.
